Please note: Intake forms will be sent to the email you provide after you book an appointment. These forms must be completed within 48 hours of receipt in order to confirm your appointment. Your appointment is NOT confirmed until the intake forms are completed and submitted. Failure to submit these forms will result in the appointment time being released. You may rebook when you are able to complete the required forms. 

I am able to accept the following insurance plans: Aetna, Anthem (Blue Cross Blue Shield), Cigna, and United Healthcare. After booking an appointment with me, you will receive an email to set up an account with Headway, which I use for insurance processing. This will be a separate email  from all other intake forms. This account must be completed at least 48 hours prior to your booked appointment time. Failure to set up the Headway account will result in the credit card you provide being charged for the full appointment fee ($250 for intake appointment, $150 for follow up appointment). To set up your Headway account, you will need your insurance information and a payment method. Your specific insurance plan will dictate your co-pay and whether your appointment fees apply towards any deductibles. Please contact your insurance plan directly if you have any questions about your coverage.
